Before use, remove the reed and bocal. Then, just drop the weighted string into the bell of the instrument, then pull the string (and the silk swab itself) through your English horn. As the silk passes through the English horn, it removes build-up from the inside of the instrument. Care instructions for the swab are a breeze, too.
